Requirements:
- Pharm Degree and a post graduate qualification in Clinical Pharmacy
- Active registration with the South African Pharmacy Council
- Five (5) years’ experience as a pharmacist (post internship) of which two (2) years must be in Clinical
- Pharmacy inclusive of Antibiotic Stewardship, Pharmacovigilance, and general Rationale Medicine Use
- Computer Proficiency in Microsoft 365 applications like Excel and PowerPoint
- Excellent communication (written and verbal) and interpersonal skills, with the ability to collaborate effectively with multidisciplinary teams

The main purpose of this position:
Our client seeking a dedicated and motivated Clinical Pharmacist to be responsible for the leading, managing, coordination, and monitoring of clinical pharmacy services and driving pharmaceutical improvement within the hospital
The clinical pharmacist will be responsible for conducting daily ward rounds and review patient medication profiles across the hospital division and contribute to improving patient outcomes
Duties & Responsibilities:
- Ensure the provision of customer focused, professional clinical pharmacy services by conducting daily ward
- rounds to perform comprehensive medication reviews for patients, monitoring responses to medications and making necessary interventions to ensure optimal therapeutic outcomes
- Ensure medication safety and timeous resolution of medication-related challenges
- Elevate antimicrobial stewardship performance as a key focus area
- Monitor microorganisms and collect and analyse Antimicrobial Stewardship data to identify inappropriate utilisation of antimicrobials
- Drive interventions to optimise drug utilisation and pharmaceutical care
- Active participation in internal stakeholder meetings including presentation of progress and gaps
- Professional collaboration by enhancing cooperative and influential working relationships with doctors, microbiologists, nursing staff, infection prevention control, pharmacists and other key functional stakeholders to achieve key outputs
- Provide evidence-based drug information and recommendations on medication therapy management, drug utilisation and formulary management
